Dear Doctor,
I am 47 yrs old. Off late I have been having severe pre- menstrual problems and my cycles are slightly irregular. I get severe back ache, head ache and pain in the knees and extreme tiredness almost ten days prior to my periods. I also have increased irritability , absent mindedness , stomach bloating and breathlessness too during these days. But my flow is ok and there is no excess bleeding. I had got my abdomen and transvaginal scanning done on the advice of my Gyanec and the following are the findings :.
1. A small fibroid in the posterior measuign 2.2 x1.3 cm
2. Bilateral small ovarian cysts with homogenous internal echoes – possibility of , chocolate cysts. The largest cysts measure 2.9x2;7 cms
The sonography itself was done with an interval of 3 months to see the growth of the fibroid and cysts.
The fibroid was at first 1.3 cms x13. cms and 1.6x1.3 and later 2.2x1.3 cm
My doctor suggest a surgery and removal of uterus so that no more complications are there. Is a surgery warranted in the first place ? . Is it possible to undergo endoscopy or is it possible to get over this problem only through medication alone ? Considering the severe pain and pre-menstrual tensions, or should I wait for menopause to set in ?What are chocolate cysts ?

Dear Doctor,
I am 48- years old and do freelance projects for IT companies. I have had two caesarian births, and an MTP and several small and big surgeries after marriage. Before marriage I was an active health enthusiast involved in cycling, swimming and had a regular , demanding job. I have hypothyroidism and take .50 mg of eltroxin every day. I am also anemic with Hb ranging between 8 to 9.5. However I am allergic to iron and hence iron supplements either give me constipation or diarhhoea . So I am trying to improve Hb through my food. But I have a very disturbing problem. For the past 7-8 years I have had body itching when I go to bed at night. Last year it became too much and I stopped using soap, replacing with our traditional besin paste , turmeric and coconut oil. What is most disturbing is that I am suffering from vaginal itching for one year now, which becomes unmanageable about ten days before my periods. I was informed by my gynec that it is due to fungal infection, especially since I am anemic and I was prescribed an antifungal oral medicine, candid as well as pessaries. But this has not helped me much. I never use public toilets, drink a lot of water and I am finicky about personal hygiene. I have regular PAP smear tests which does not show much except slight inflammation and mammogram. I also take Vitamin E, calcium supplements and B Complex. Apart from a horde of peri-menopausal symptoms, this problem is too embarrassing and inconvenient. Is there a solution to this ?

I had a severe injury in my left foot a few months back. After prolonged treatment, the wound has healed now. Every now and then I get a pricking sensation in my foot. I am also afraid to go for walks, for fear of straining my leg. I am sixty years old and a diabetic for more than ten years. How do I take care of my wounded foot? Can I go for long walks? I am an active, healthy senior citizen and still employed as a consultant.

I think that you should definitely go for walking after using proper footwear to prevent injuries. Walking can improve the blood circulation to the feet as well as the nerve sensation. You should also keep your sugar under good control.
